《财经》实习记者 程晗 记者 李虎军 [11-13 10:38] http://www.caijin... 初步研究成果支持经穴特异性理论,但临床效果和生物学基础方面仍待深入研究 【《财经网》专稿/实习记者 程晗 记者 李虎军】历史悠久的针灸,如今在海内外都得到了广泛应用。但学术界迄今为止仍不清楚,针灸疗效到底是如何产生的。一个获得上千万元经费支持的中国研究团队,已经开始探索针灸疗效之谜。 日前,在北京举行的世界卫生组织传统医学大会“针灸与人类健康”卫星会上,这个团队的首席科学家、成都中医药大学副校长梁繁荣报告了180例偏头痛病人临床试验的结果,并初步证实针灸疗效的关键在于经穴特异性。 在整个中医体系中,针灸无疑最具国际化色彩。1972年,随尼克松访华的《纽约时报》记者詹姆斯·雷斯頓(James Reston)在手术之后,接受了针灸治疗,以缓解疼痛。回到美国之后,雷斯頓用一支妙笔介绍了这段经历,针灸也随之走出国门。 随着海外针灸热潮的兴起,美国、德国等发达国家相继投入资金,开始了针灸疗效的临床研究。 其中,美国马里兰大学医学院的研究人员2004年12月在《内科学年鉴》杂志上报告了570位膝关节炎病人用针灸持续治疗半年的临床试验结果:针灸确实可以镇痛,同时改善膝关节炎的运动功能。这也是国际上首次有大规模随机对照临床试验(RCT)证实,针灸可以帮助治疗膝关节炎。 在现代医学中,验证某种新药的办法通常是进行大规模随机对照临床试验。也就是说,随机挑选相当数量的病人,一组接受治疗,另一组则服用貌似新药的安慰剂,作为对照。医生和病人都不知道具体的分组情况,称为“双盲”。 “双盲”的原则很难用于针灸。为了瞒过病人,可以考虑给对照组假扎针——针并不真正扎入,或者故意不扎在穴位上。显然,这种方式很难瞒过医生,只能称为“单盲”,但仍然为国际学术界所接受。 在中医体系中,指导针灸临床的主要理论是经络学说。该学说认为,只有在选取恰当穴位的前提下,针灸才可能发挥良好的治疗效果。换句话说,针灸的关键在于穴位功能主治的特异性。 不过,德国、荷兰等国学者对偏头痛、慢性顽固性哮喘、下背痛、针灸戒烟的针灸临床研究结果,却得出了无效的结论,给经穴的特异性提出了质疑和挑战。 梁繁荣教授告诉《财经》记者,这大概是由于国外一些研究机构对中医理论的认识不深,研究方法上也存在差距,造成许多针灸临床研究结果得出了无效的结论。 而令人尴尬的是,中国在针灸临床评价等方面长期缺乏投入。直到2006年,中国针灸研究领域的第一个国家重点基础研究发展计划(973计划)项目才获得批准,研究经费共1400万元。项目参与单位包括成都中医药大学、华中科技大学、北京中医药大学、天津中医药大学第一附属医院、广州中医药大学和中国中医科学院,由梁繁荣担任首席科学家。 在世界卫生组织传统医学大会“针灸与人类健康”卫星会的主题发言中,梁繁荣报告了这一研究项目的进展。 首先,研究人员分析了古代和现代的数千条文献,显示经穴效应具有特异性,且规律为“经脉循行是基础,经气会聚是关键”。 此外,研究团队开展了国际公认的多中心大规模随机对照临床研究。来自三个研究中心的七家医疗机构,共纳入180例偏头痛急性发作期的患者。所有患者都被随机分入针刺组和对照组。对于针刺组,选择风池、角孙、外关、阳陵泉、丘墟穴进行治疗;而对照组则采用非经非穴的“假针刺”。 结果表明,针刺经穴治疗偏头痛急性发作期的疗效,比假针刺的疗效好,其优势体现在针刺后两小时和四小时,并且防止头痛复发和加重的效果明显。 研究人员还利用前期动物实验和小样本的临床研究,对经穴效应特异性的生物学基础进行了初步探讨。其中,针对偏头痛和功能性消化不良患者的PET/CT脑部扫描结果提示,经穴针刺特定的反应较非经穴显著;而基于核磁共振技术的代谢组学研究方法则显示,经穴针刺对机体的代谢产物影响较非经穴显著。 当然,梁繁荣教授对《财经》记者强调,该报告只是初期的研究成果,下一阶段将在经穴特异性的临床效果和生物学基础方面进行更深入的研究。■Shared by qInCI (131)Contribute comment - Stay Faithful to Core Values - WSJ.com (1)

RouteNote is a music distribution startup in the UK which offers unsigned or independent artists from anywhere the ability to get their music distributed to a bunch of online stores. Artists upload tracks to the RouteNote catalogue and select partnered retail outlets that they wish their music to be available through (e.g. Napster, Snocap, Samsung Mobile). That's the claim made by Paul Boutin, Editor, Wired magazine, who said: "Cut-rate journalists and underground marketing campaigns now drown out the authentic voices of amateur wordsmiths."I'm not sure I agree.
